Our memory and dementia care clinic supports patients and families through every stage of assessment, diagnosis, treatment planning, caregiver support, and ongoing care.

Patients may be referred to the clinic for assessment of memory or cognitive concerns, diagnostic clarification, medication review, and ongoing follow-up care related to dementia care. Our team works in collaboration with your family physician, keeping them informed of diagnosis, treatment plans, and new developments.
We provide assessment services, medication planning where applicable, and behavioural care strategies. Furthermore, our Health Services Coordinator conducts a home visit and collaborates with community programs to create personalized support plans that connect clients and families with resources, promote autonomy, and help maintain dignity and quality of life while reducing caregiver burnout.

Dr. Maiza is a family physician with a special interest and experience in Geriatric Medicine, as well as Memory and Dementia Care.
Dr. Maiza provides memory and cognitive assessments and works closely with our Psychiatrist and our Health Services Coordinator to support accurate diagnosis and personalized care and medication plans. He also collaborates with your family doctor to keep them informed of diagnoses, treatment plans, and any new developments.
Dr. Maiza brings broad clinical experience from practising in Nigeria, Saudi Arabia, the U.K., and Canada. His extensive experience allows him to work effectively with patients from a wide variety of socio-cultural backgrounds, ensuring respectful and understanding communication. With a primary goal of delivering the best possible care to all his patients, Dr. Maiza is dedicated to building long-term, trusting relationships within the community. He is able to provide services in English and Arabic.

Dr. Mackenzie Acheson is a Psychiatrist with a special interest in supporting older adults with cognition, and mental health needs that often accompany memory concerns and dementia. He works closely with our family physicians and care team to help with diagnostic clarification, behavioural and psychological symptoms of dementia, mood and anxiety concerns, caregiver support, and individualized treatment planning.
Born and raised in Winnipeg, Dr. Acheson is proud to serve the community that shaped him. His commitment to dementia care is both professional and personal. Having seen the impact of dementia in his own family, as well as through years of volunteer work in elder care facilities, he brings a grounded understanding of how memory disorders affect patients and the people who care for them.
His approach emphasizes practical, compassionate, and collaborative care that supports patients and families through every stage of the diagnostic and treatment process. He is able to provide services in English and French.

As the Health Resources Coordinator, Marika is passionate about supporting individuals and families navigating the challenges of memory loss and cognitive decline. She brings compassion, advocacy, and practical problem-solving to every interaction.
A graduate of the University of Manitoba with a degree in Social Work (2007), Marika spent 12 years as a WRHA Home Care Case Coordinator and 5 years supporting families through the Self and Family Managed Care program. Her hands-on experience gives her a unique perspective when helping clients navigate Manitoba’s complex home care and funding systems.
Marika works closely with clients, families, and Dr. Maiza and Dr. Acheson to create personalized care plans that promote dignity, comfort, and quality of life. Whether connecting families to community resources, offering emotional support, or helping clients maintain a sense of independence, Marika approaches each person with empathy and respect.
Step 1: Diagnosis and Assessment
Assessment for memory concerns may include cognitive testing, review of medical history and medications, and discussion of any changes in day-to-day function. As part of a comprehensive evaluation, the care team works collaboratively, including case review with the psychiatrist, to help clarify the diagnosis and guide personalized care recommendations.
Step 2: Medication Review
As part of the assessment process, medications are carefully reviewed to ensure they are appropriate and optimized for the patient’s diagnosis and overall health needs. This includes identifying medications that may affect memory or cognition, as well as any potential interactions with dementia treatments. Recommendations are then shared with the referring physician for review and follow-up.
Step 3: Home Visit
Our Health Services Coordinator Marika Sylvestre conducts a home visit to better understand each client’s needs, strengths, and goals, and works with community programs to develop individualized support plans. She also connect families with community resources, respite services, housing options, and financial supports to help reduce caregiver burnout, while promoting meaningful engagement, autonomy, dignity, and quality of life for each patient.
Step 4: Follow-Up
Patients may be offered follow-up appointments to monitor how memory or dementia treatments or medications are working and to adjust the care plan as needed. Follow-up care may also include ongoing communication with the patient’s primary care provider to support coordinated and continuous care.
